Annually, a public school district shall participate in the
Division of Elementary and Secondary Education health services survey related
to the following:
- On-campus healthcare needs of public school students;
- On-campus healthcare services; and
- The availability of health care to other public school district personnel and nursing facilities.
Each public school district shall report the information
reported in the health services survey to the local school district board of
directors.
The 2021-2022 DESE Health Services Survey must be completed and
submitted online by all school nurses. The survey linked below is open through June
10, 2022. A printable PDF file of the survey can be downloaded for
information gathering purposes. However, electronic submissions outside the
Google survey form will not be accepted. In an effort to capture the most
accurate information possible, districts are encouraged to complete the survey
during the last week of on-site instruction for students this school year.
